Network Meta-Analysis of Hepatic Arterial Infusion Versus Selective Internal Radiation Therapy for Colorectal Cancer Liver Metastasis

1. Abstract 1.1. Background: Colorectal cancer liver metastases (CRLM) that cannot be surgically removed are often addressed locally with Hepatic Arterial Infusion (HAI) or Selective Internal Radiation Therapy (SIRT). This study used Bayesian network meta-analysis (NMA) to assess the differences in survival between patients treated with HAI or SIRT for CRLM.

Rare Case of Gastrostomy Tube Migration Abutting Ampulla of Vater and Evidence-Based Treatment of the Resulting Pancreatitis

1. Abstract This case demonstrates a rare cause of acute pancreatitis secondary to migration of a gastrostomy tube into the descending part of the duodenum to cause pancreatic duct obstruction. Gastrostomy tubes are considered first line and preferred method in providing long term enteral nutrition in those who have functional gastrointestinal tracts however insufficient oral …

Determinants of the Outcome in Liver Cirrhosis Patients Admitted in Intensive Care Unit for Severe Sepsis or Septic Shock: An Unselected Cohort Study

1. Abstract 1.1. Introduction: Determinants of mortality of patients with liver cirrhosis and septic shock are poorly known. 1.2. Methods: Multicenter observational study including all patients with liver cirrhosis admitted in ICU for severe sepsis/septic shock in 1997-2014 from the OUTCOMEREA® database. Patients with liver transplantation were excluded. Risk factors of 28-day and 90-day mortality …

Hepcidin, Ferroportin and Hephaestin Levels in Inflammatory Bowel Disease; Are they Key Mediators of Anemia or Inflammation?

1. Abstract 1.1. Background: Anemia represents a significant problem in Inflammatory Bowel Disease (IBD) and diagnosis may be challenging due to inflammation. Hepcidin, ferroportin and hephaestin are novel Iron Regulatory Proteins (IRPs) involved in iron metabolism. This study aimed to identify the interactions of these proteins in IBD patients, guiding safe iron supplementation and effective …

Giant Hepatic Hematoma after Endoscopic Retrograde Cholangiopancreatography (ERCP): A Rare Etiology of Hemorrhagic Shock

1. Clinical Image A 38-year-old bile prosthesis bearer, not anti-coagulated nor anti-aggregated, was subjected to an ERCP to move it away. A hard-working procedure was made so multiple attempts were needed. After it, the patient felt right lumbar pain and hemoglobin fall, developing hemorrhagic shock and requiring orotracheal intubation, blood components transfusion and use of …

Non Langerhans Cell Histiocytosis of the Rectum: Case Report and Review of the Literature

1. Abstract Histiocytosis is a condition resulting from the abnormal proliferation of dentritic cells of the mononuclear phagocytic system. Previously classified as Langerhans cell histiocytosis and non-Langerhans cell histiocytosis, and recently into 5 categories (L, C, R, M and H), it can be localized to an organ or have a systemic manifestation, affecting most commonly …

Unusual ERCP Complication: Unwanted Course of Misdirected Bile

1. Abstract Biliary stent complications include stent migration, obstruction, fragmentation, bleeding, perforation, cholangitis, and post-ERCP pancreatitis. Stent migration is the most common adverse event (5-14%) and is associated with: dilated CBD >10mm, benign stricture etiology, concomitant sphincterotomy, stent duration >1 month, and type of stent placed (straight, 10Fr diameter, and length >9cm). Here, we present …
